ABSTRACT

Objective: To study the protective effect of Bisphosphonates [Pamidronate] in patients of primary osteoporosis in Pakistani setup. Study Design: Quasi experimental study. Place and Duration of Study: The study was carried out at department of orthopedic and spine surgery, CMH Rawalpindi, from Sep 2011 to Sep 2013


Material and Methods: A non probability convenience sample was collected from 50 patients who visited orthopaedics department with complaints of generalized aches and pains and diagnosed to have primary osteoprosis. Osteoporosis was confirmed by DEXA scan, with a T-score of more than -2.5. Pamidronate-a bisphosphonate, was given I/V on monthly basis along with oral calcium. After 6 month and one year DEXA scan was repeated to observe its effect on bone mineral densit


Results: Osteoporosis T score of DEXA scan was seen to improve by 19.3% [0.193 T score improvement] with a p-value less than 0.05


Conclusion: Pamidronate treatment increased bone mineral density in osteoprotic patients

ABSTRACT

To determine the frequency of Hirschsprung's Disease as a cause of neonatal intestinal obstruction, to compare the results of treatment with others and to suggest means and ways to improve the deficiencies in our circumstances. Nishtar hospital and Ibn-e-Siena Hospital, Multan from January, 2008 to June, 2012. A total number of 115 patients from neonatal age to more than 5 years were diagnosed and treated for Hirschsprung's Disease. Hirschsprung's Disease was a cause of neonatal obstruction in 26 [22.6%] patients. Hirschsprung's Disease was a cause of recurrent diarrhea in 37 [32.17%] patients. 76 [66.08%] patients presented with chronic constipation. 27 patients underwent for pull-through operation. The overall complications rate and stooling pattern after Soave's procedure are not significantly different from others

ABSTRACT

This study involves the review of management of patients who required temporary airway support and were treated either with tracheostomy or endotracheal intubatione in ear, nose, throat. Department and intensive care unit of Multan Medical and Dental College Ibne Sina Hospital Multan. Another important aspect of the study is to compare the results of tracheostomy with endotracheal intubation. Retrospective and descriptive. Multan Medical and Dental College Ibnae e Sina Hospital Multan. From 2012 to 2013. Twenty consecutive patients who had operation of tracheostomy whether emergency or elective, and were admitted in ENT ward of Ibne Sina Hospital Multan and twenty patients were intubated with endotracheal tube in the intensive care unit of the hospital. The main indications of tracheotomy were upper air way obstruction [80]% and mechanical respiratory insufficiency [20]%. Indications of endotracheal intubation were mechanical respiratory failure[80]% and ventilatory insufficiency due to retention of secretions [20]%. Tracheostomy is better choice in prolonged air way control more than three 3 weeks. But endotracheal intubation is found to be better choice in early and quick management of short term airway control up to 3 weeks

ABSTRACT

To determine the frequency of Hirschsprung's Disease associated enterocolitis in pre and post operative patients. Nishtar Hospital and Ibn-e-Sina Hospital, Multan from January 2008 to June 2012. A total number of 114 patients aged from less than 6 months to more than 5 years were diagnosed and treated for Hirschsprung's Disease after rectal biopsy and internal sphinterotomy. In total of 114 patients 32 [28%] developed enterocolitis before and 5 [18.5%] after definite surgery. The results of this study implicates that the early diagnosis and to make measures to decrease the post pull-through anastomostic anorectal stenosis can reduce the frequency of pre and post operative Hirschsprung's disease associated enterocolitis

ABSTRACT

To evaluate protective effect of ascorbic acid on liver parameters in mice exposed to Cypermethrin. Laboratory based randomized control trial. Research was conducted in Army Medical College's biochemistry and molecular biology department in association with department of pathology, Army Medical College, Rawalpindi and National Institute of Health, Islamabad, Pakistan from 19 May 2013 to 17 June 2013. Thirty albino mice of Balb/C strain weighing 40-45 g were randomly divided into three groups. Each group comprised 10 mice. Control group A which received normal diet. Cypermethrin experimental group B received Cypermethrin with normal diet experimental group C which received Cypermethrin and vitamin C with normal diet. This process continued for 28 days. After this duration serum alanine aminotransferase [ALT] and serum aspartate aminotransferase [AST] values were determined. Serum ALT and AST levels were significantly increased in group B as compared to group A [p < 0.001]. ALT levels of group A and group C were insignificant [p = 0.473]. AST levels of group A and C were significantly different [p < 0.01]. Ascorbic acid can protect liver from Cypermethrin induced oxidative stress in mice

ABSTRACT

To assess the nutritional status and potential risk factors for malnutrition in children less than 5 years of age in a community. Basic Health Unit at Nawanshehr, Abbottabad from December 2008 to June 2009. A community based cross sectional study was done over 06 months. Random cluster sampling was done and sample size of 393 houses was calculated with an error of 10%. The parameters studied in children were age, weight, nutritional status and weaning practices while for mothers the parameters included their literacy and knowledge about their health. WHO's criteria of malnutrition - NCHS was used. Data were entered in SPSS 15.0 and analyzed. A total of 393 children and their mothers were included in the study. In children; nutritionally, 245[62.3%] were normal, 148 malnourished, 77[19.6%] undernourished and 71[18.19%] over nourished. When data of normal children was compared with malnourished and it was found that literacy of mother, her knowledge about health including hand washing and maintaining contact with lady health worker, boiling water and giving ORS and practice of not throwing garbage at open spaces were significant predictors of better child nutritional status. Child's nutritional status is strongly associated with the literacy of mothers, their health awareness and child rearing practices

ABSTRACT

To find out the frequency of microalbuminuria in patients with diabetes mellitus both type 1 and 2 with reference to glycemic control and duration of disease. Study was conducted at Dr. Ziauddin University Hospital North Nazimabad Campus, Karachi. Fifty seven diabetic patients of either sex were included in the study. Patient taking Angiotensin Converting Enzyme [ACE] inhibitors were excluded from the study. Out of 57 diabetic patients, 22 [38.6%] patients had HbA1c less than 7.0% and 35 [61.4%] patients had HbA1c more than 7.0%. Out of 22 patients with optimal glycemic control 8 [36.4%] had microalbuminuria, 3 [13.6%] had clinical albuminuria and 11 [50.0%] had no microalbuminuria. While out of 35 patients with poor glycemic control 10 [28.6%] had microalbuminuria, 9 [25.7%] had clinical albuminuria and 16 [45.7%] had no microalbuminuria. Out of 57 patients; 14 [24.6%] were having disease up to 5 years. Out of them 10 [71.4%] had no microalbuminuria and 4 [28.6%] had micro and clinical albuminuria. Patients with disease duration between 6-10 years were 18 [31.6%], out of which 10 [55.6%] had no microalbuminuria and 8 [44.4%] had micro and clinical albuminuria and 25 [44%] patients were having disease duration more than 10 years, out of these, 7 [28%] had no microalbuminuria and 18 [72%] had micro and clinical albuminuria. Diabetic patients with increased duration of disease are more prone to renal complication irrespective of glycemic control at the time of study. Therefore besides monitoring for their glycemic control, these patients should be monitored for development of nephropathy especially more frequently as the disease duration progresses.
